Frequently Asked Questions
Why am I getting this bill? I already received a bill from the hospital for my ER visit.
When you go to the hospital emergency room, there are two bills generated for your visit. One is for the hospital, they charge for the facility fees such as your x-rays and lab work, as well as the cost of maintaining the facility itself. The physician charges are separate. The bill you received from EPPA is for the physician’s time as well as any procedures the physician performed while you were in the ER.
Will I have to pay a co-pay or deductable for my emergency care?
All benefit plans are different. You will want to check with your individual carrier to determine what benefits will be applied to your ER visit. Click here for a list of insurance company websites and phone numbers.
Do I need prior authorization so my insurance will pay?
Some health plans require notification of an emergency visit within 24 hours to ensure coverage. This varies by individual carrier; you will want to check with your health plan to determine your notification requirements. Click here for a list of insurance company websites and phone numbers.
What kind of insurance do you take?
We will bill any health insurance company for your service. After the insurance company makes their decision about what they are going to pay, we will send you a bill based on the payment amount from your insurance company. We have contracts with many commercial health plans and we accept all state Medicaid programs as well as Medicare.
What is HIPAA?
HIPAA is the Health Insurance Portability and Accountability Act of 1996. Click here for further information.
Why can’t you talk to me about my husband/wife’s bill?

Please click here for a printable form you can fax us so that we can document your accounts. You can also have your spouse call us at 952-857-9880 to verbally authorize you to speak with us.
I need a copy of my medical records. Where do I get those?
Although EPPA receives a copy of your medical record for billing purposes, the actual record is housed at the hospital. Please click here for a list of the hospital medical record department contact information.
